Shellfish is the most common food allergy in American adults, and it breaks the usual food-allergy rules. Most food allergies start in childhood; shellfish allergy famously shows up out of nowhere in your twenties, thirties, or later, often after years of eating shrimp without a problem. It is also one of the least likely allergies to ever leave. Here is how to recognize it, what the rash actually looks like, and what treatment can and cannot do.
What a shellfish allergy reaction looks like
Symptoms usually begin within minutes to two hours of eating: hives, itching or tingling in the mouth, swelling of the lips, face, or throat, stomach pain, vomiting or diarrhea, congestion, and in severe cases wheezing, throat tightness, dizziness, or full anaphylaxis. Shellfish is one of the leading causes of food-related anaphylaxis in adults, which is why diagnosed patients carry epinephrine everywhere. Some highly sensitive people react to cooking steam alone; a seafood boil can be a problem before anything touches the plate.
What the rash looks like
The classic shellfish allergy rash is hives (urticaria): raised, intensely itchy welts with pale centers and red borders that look and feel like mosquito bites gone wrong. They can be a few isolated bumps or large merged patches, and they migrate, fading in one spot while new ones appear somewhere else over hours. They typically show up on the face, neck, chest, and arms within minutes to two hours of eating. Two features separate an allergy rash from ordinary skin irritation: the timing, tightly linked to the meal, and the itch, which is severe. A rash that appears alongside lip or face swelling, stomach symptoms, or any breathing change is an emergency, not a skin problem.
Crustaceans vs. mollusks: which shellfish are the problem?
Shellfish splits into two families, and the distinction matters clinically. Crustaceans, shrimp, crab, lobster, and crawfish, cause most reactions and most severe reactions; the main culprit is a muscle protein called tropomyosin. Mollusks are the other branch: clams, mussels, oysters, and scallops, plus, and this surprises people, squid, octopus, and snails. So the calamari question has a real answer: calamari is squid, squid is a mollusk, and it absolutely counts as shellfish. Many people react to only one family, some react to both, and an allergist can test each so you are not avoiding half a menu on guesswork. U.S. labeling law requires crustacean shellfish to be declared on packaged food; mollusks, oddly, are not covered by that rule, one more reason a precise diagnosis beats blanket caution.
Why it starts in adults, and whether it is genetic
Roughly half of shellfish-allergic people had their first reaction as adults, which makes it the signature adult-onset food allergy. Nobody fully knows why, but one clue is dust mites: their tropomyosin is structurally similar to shrimp tropomyosin, and years of dust mite exposure may prime the immune system to react to shellfish. Family history matters too: having a parent or sibling with shellfish allergy raises your risk, though there is no single gene and no guarantee in either direction. What is well established is persistence: shellfish allergy rarely goes away. Unlike egg or milk allergy, which most children outgrow, shellfish allergy is usually a lifelong condition once it appears, in children and adults alike.
The iodine myth
A stubborn piece of medical folklore says shellfish-allergic people are allergic to iodine and must avoid iodine-based contrast dye before CT scans. It is false. Shellfish allergy is a reaction to shellfish proteins, tropomyosin chief among them, not to iodine, and studies show shellfish-allergic patients react to contrast dye at roughly the same rate as people with any other allergy. Tell your care team about every allergy you have, but do not let anyone cancel your scan over shrimp.

Treatment: what exists today
The core management is unglamorous: strict avoidance, label reading, restaurant vigilance, and epinephrine within reach at all times. People allergic to crustaceans do not automatically react to mollusks, so some shrimp-allergic diners tolerate calamari, but the reverse is also true and cross-reactivity is common enough that an allergist should make that call through component testing, never a restaurant experiment.
